        最新的科研證據顯示,心理疾病的根源可能在於腸道微生物,多數心理疾病可能本質上是胃腸道疾病,針對腸道微生物來治療心理疾病,可能是未來精神病學發展的有力方向。而能夠改善情緒和認知,給精神疾病患者帶來益處的好細菌,也被稱之為益心菌(psychobiotics)。下面將簡單介紹躁鬱症和躁狂症與腸道微生物的關係。

 

 

 

4.1 躁鬱症和躁狂症與腸道微生物

 

 

        雙相障礙,也稱雙相抑鬱障礙,最早被稱為躁狂抑鬱症,是一種以情感的異常高漲和低落為特徵的精神障礙性疾病,其病因尚不明確,兼有躁狂狀態和抑鬱狀態兩種主要表現,可在同一病人間歇交替反復發作,也可以一種狀態為主反復發作。雙相障礙患病率約為6%,患者初次尋求治療時常因為抑鬱症狀而忽略躁狂症狀,可能被誤診為單相抑鬱,往往數年之後才會確診[8]。

 

 

        躁鬱症在青年女性中高發,原因與環境或情緒狀態變化幅度較大,暴飲暴食等有一定關係。在本研究室收集觀察和研究到病例當中,由於離開家庭,走上社會和出國留學等情況居多。由於大幅度的飲食習慣變化,生活環境變化,異國文化氛圍的變化,以及學習壓力和家庭不切適的干預等等都可能是誘因。

 

        躁鬱症與菌-腸-腦軸異常密切相關,針對腸道微生物,調節菌-腸-腦軸功能可能是治療躁鬱症的好方法[9, 10]。

 

 

1. 躁鬱症患者的腸道微生物明顯不同于對照健康人群[10-12]。患者菌群多樣性降低[13],菌群組成與疾病嚴重程度相關[11]。

 

 

 

2.   除了心理精神症狀,患者還可能出現腸漏、免疫異常等與菌群紊亂密切相關的胃腸道症狀[9,14, 15]。

 

 

3. 抗生素破壞菌群會增加躁鬱症風險[15]。

 

 

4. 補充益生菌,調節腸道微生物,對患者具有改善作用[12,15]。

 

 

 

 

 

4.2躁狂症與腸道微生物

 

        躁狂症是指一種在一段明確的時間內有異常而持續的心境高漲、誇大或易激惹為特徵的精神疾病,躁狂也是雙相障礙的一種發作形式。

 

        雖然躁狂症與腸道微生物的研究不多,但已有的研究提示,腸道微生物異常在躁狂發生中發揮了不容忽視的作用,在躁狂症治療中考慮腸道微生物調整可能會取得更好效果[16-18]。金鋒實驗室動物實驗表明,使用抗生素的動物腸道裡往往產生含量較高的硫化氫和氨,這些氣體神經遞質對於動物的躁狂易激有著不可忽視的作用。

 

1. 抗生素破壞菌群可能會引起躁狂症狀[19]。

 

2. 活性炭吸附藥物保護菌群可減輕躁狂症狀[17, 20]。

 

3. 腸道微生物組成影響躁狂症等心理疾病藥物治療的效果[21]。
